From arts and crafts to rehabilitation, Horizons Adult Day Heath Care Center offers a wide variety of services and activities.
Assessments
Our multidisciplinary team members conduct a comprehensive evaluation of each new participant and recommend support services based on their specific needs, goals and desired outcomes. We invite the participant, family, caregiver, and/or individuals of the participant’s choosing to fully participate in the service planning process. This collaborative and recurring process is part of our ongoing commitment to the participant.
Nursing Services
Our professional team of nurses coordinates care, manages medications, observes and monitors participants’ health status and any changes in their condition. The individual medical, cognitive or mental health condition and risk factors are taken into consideration. In addition, our nursing staff works in partnership with the participant, family and/or caregiver, and primary physician to coordinate and provide all care.
Personal Care Assistance
Our caregivers provide continuous care under nursing supervision to ensure participants’ safety and protection. Assistance is provided with walking, grooming, dressing, eating and all personal hygiene needs including toileting and continence care. Cleanliness and safety are essential to our quality of care.
Therapeutic Activities
Daily group and individual activities are provided to enhance the social, physical and cognitive functioning of the participant. Person-centered activities are designed to be fun, stimulating, educational and entertaining. Facilitated participation is provided to those whose frailty or level of cognitive functioning impedes them from active participation.
Social Services
Our team of social workers meets with each participant upon admission to assess their social and psychological needs. Social workers serve as a liaison with the participant’s family, home and other community agencies. In addition, the social worker provides a home safety evaluation and meets with each participant on a regular basis. Our Social Workers provide a personal touch to each of our participants to keep them connected to the community and make them feel like family.
Restorative Therapy
Our physical, occupational and speech therapists provide rehabilitation care using the latest proven techniques. Our team approach, including therapists, nurses and the participant’s physician, allows for more continuity of care from all perspectives. Occupational, Speech, and Physical Therapies are available to address individual needs.
Dietary Services
Keeping our participants healthy, inside and out. Our kitchen staff prepares appetizing, tasty and nutritious meals daily. Mealtimes are also the source of social connections – a time to converse with other participants. We serve breakfast until 10:00 followed by a hot lunch at 12:00 and a tasty snack at 2:00. Our Registered Dietitian plans all menus, ensuring balanced and safe meals that accommodate and meet the nutritional needs of the individual.
Transportation Services
Our safe and reliable transportation team will be there for you when arriving and departing our facility. Transportation is provided to and from the participant’s place of residence at no additional cost. Our professional and courteous drivers operate a clean, well-maintained wheelchair-equipped fleet providing unparalleled access to the community.
Psychiatric/Psychological Services
Services are provided for the treatment or stabilization of a diagnosed mental disorder by a licensed mental health professional on staff. Individual and group counseling is provided which aims to help individuals improve their health and wellness, live a self-directed life, and strive to reach their full potential as a member of the community.
Horizons ADHC participates in the Child and Adult Care Food Program (CACFP).
