From arts and crafts to rehabilitation, Horizons Adult Day Heath Care Center offers a wide variety of services and activities.

Horizons Adult Day Health Care Center is a Licensed Community Based Adult Services program (CBAS) that provides services to the elderly and adults with illnesses and or disabilities.

Our program is designed for physically and/or cognitively impaired adults to provide a structured day program. Independence, self-expression and being part of a group helps adults, who may be isolated by illness, disability or loss, maintain a positive orientation on life. By focusing on maintenance or physical and mental health, we enable participants to maintain as high a level of functioning as possible, thereby allowing them to remain in their homes and communities as opposed to being institutionalized.

Our Team

Our professional and highly skilled staff is comprised of degreed, licensed and/or certified clinicians and practitioners in the fields of social work, nursing, nutrition and physical, occupational, and speech therapy. Horizons Adult Day Services’ staff is ethnically diverse and multilingual in English, Tagalog, Spanish, Vietnamese and Cantonese.

Horizons full-time program staff includes professional social workers, registered nurses, licensed vocational nurses, therapy aides, certified nurse assistants, activities coordinator and aides. All of our caregivers are well-trained, compassionate and have worked with us for many years.

Our program consultants include a physician, physical therapist, an occupational therapist, speech therapist, psychologist, registered dietitian and a pharmacist.

We are prepared to meet the individual needs of our participants and their families by providing respite care, counseling, referrals, daily socialization, and rehabilitation. Our program is designed to prevent participants from being institutionalized and allow them to maintain their personal independence. We make it a top priority to treat all participants with dignity and respect.

Kimberly Zimmerman – Administrator

Kimberly graduated from the University of Texas at El Paso where she earned her degree of Bachelor in Business Administration in 1998. She has worked in the field of adult day health care since 2002. She also has experience in ICU, rehabilitation, and ER in various hospital settings. Kimberly brings compassion and understanding as Administrator.

Michael J. DuLaney, BSW – Program Director

Michael graduated from Eastern Michigan University where he majored in psychology and sociology. Since 1994, he continuously worked with seniors, adults with disabilities, and those with chronic mental illness in a variety of settings including: skilled nursing facilities, geriatric counseling and recently adult day health care centers. His work experience provides Horizons with an exceptional Program Director.

About Adult Day Health Care and Community Based Adult Services?

Adult Day Health Care (ADHC) also known as Community Based Adult Services (CBAS) is a licensed Medi-Cal certified health facility that treats the health and supportive needs of adults with multiple, chronic conditions in a safe, homelike day setting. ​

These facilities aren’t just health-focused. Socialization is important and a key factor in the holistic approach to healing and caring. ADHC provides participants with opportunities to participate in a variety of different individual and group activities.

The goal of ADHC is to prevent or delay placement into other more expensive care settings. This is done by improving and preserving each individual’s physical and mental health, improving their quality of life. Adults with chronic conditions are able to successfully live in the community while a benefit for the caregiver is regular respite from 24-hour caregiving responsibility.

ADHC/CBAS promotes optimal health and mental well-being for the participant, compliance with physician treatment plans, support for the caregiver, in a supportive, enjoyable setting.

ADHC/CBAS is a bargain compared to purchasing these services separately!
