Welcome to Horizons Adult Day Health Care Center

Horizons Adult Day Health Care Center is a Licensed Community Based Adult Services program (CBAS) that provides services to the elderly and adults with illnesses and or disabilities.

This program is individualized for each participant, based on his or her needs and plan of care. Independence, self-expression and being part of a group helps adults, who may be isolatedby illness, disability or loss, maintain a positive orientation on life.

By focusing on maintenance of physical and mental health, Horizons ADHC enables the participant to maintain as high a level of functioning as possible, thereby allowing them to remain in their homes and communities as opposed to being institutionalized.

We strive to bring a balanced approach to all care provided by listening to and attending to each individual’s unique personal needs.

Families Need a Break Too

Caring for a loved one with a debilitating disease such as Alzheimer’s Disease, Stroke or Parkinson’s Disease, requiring constant attention and supervision is exhausting! Using CBAS to supplement the care you provide at home allows the family/ caregiver respite time. CBAS also allow their loved ones to remain at home as long as medically, socially and economically feasible.

Blood Pressure can be monitored daily when your loved one is at the center.

Eligibility for CBAS

Individuals may be eligible for CBAS if they meet one or more of the following criteria:

  1. Individuals who meet Nursing Facility Level of care.
  2. Individuals who have an Organic, Acquired or Traumatic Brain Injury and/or Chronic Mental Illness.
  3. Individuals with Moderate to Severe forms of Alzheimer’s Disease or other Dementia.
  4. Individuals with Mild Cognitive Impairment including Moderate Alzheimer’s Disease or other Dementia.
  5. Individuals with Intellectual Disabilities and Meet the Criteria for San Diego Regional Center Eligibility.

CBAS Objectives

  1. Rehabilitate and/or maintain participant’s level of care to the maximum extent possible.
  2. Maintain the participant in the community for as long as possible.
  3. Prevent inappropriate or premature institutionalization.
  4. Assist participant’s family/caregivers by providing respite care to their loved ones.

  • San Diego Regional Center
  • Private Pay

For persons not covered by the above plans, please contact the center’s out reach coordinator worker to discuss payment options.
